Facial plastic surgeries:-

Changes occurring with age include dryness of skin, development of dys (altered) pigmentation, freckles, wrinkles, localized telangictasia, thinning of dermis deepening of facial crease lines and sagging of entire face.

Both skin and muscle layers are tightened to provide face a natural youthful appearance.
Face lift is often combined with neck lift and brow lift .

Forehead and Periorbita :- Wrinkle line becomes more prominent with descent of brow, giving face a tired look and sense of heaviness around eyes.

Brow Lift gives forehead a fresh appearance and also reduces sense of heaviness around eyes.
Eyelid bagginess also develops with age due to loss of proper support to orbital fat and the later protrudes. In blepharoplasty, excess skin and muscle is taken out and fat is redistributed to provide youthful appearance.

In our population, brow lift should be considered first followed by Blepharoplasty or Eye lid Surgery as in most cases bagginess is due to brow descent rather than eyelid.

Crow feet:- sun ray type radiating wrinkle line at lateral ( outer canthus) of eye. Can be corrected with fillers, Botox or Blepharoplasty or Eye Lid Surgery with Lateral Canthoplasty.

Upper & Lower eyelid bagginess ( Blepharoplasty ) :- again due to loss of support and descent of fat. Lower lid Blepharoplasty tightens Musculo-Cuteneous apron and redistributes fat.

Eyelid Canthoplasty :- Medial ( Inner ) & lateral (Outer) Attachments of eyelid to bone are called Canthus. They provide fulcrum for eyelids to move and also important for Lacrimal pump mechanism.  Laxity in eyelid  leading to failure of normal lacrimal pump mechanism & excessive lacrimation may be due to detachment of canthi ( mostly traumatic ) or laxity of eyelid support (as in old age) or lack of globe support to eyelid (as in sunken eyes - post traumatic Fracture of orbital floor or loss of fat in orbit (as in Myopic ( people with negative power glasses ) people .

Deepening of Nasolabial and Melo Labial Fold:- the line between nose and cheek and lip and cheek. Minor ones can be addressed with fillers , for major ones with face descent- face lift is best option.

Fillers:- May be artificial like collagen , hyaluronic acid etc. or autogenous ( fat or dermis)
Cheek depression:- Augmentation with fat injection to reduce prominence of acne scar along with cheek augentation.      Back to Top

Wrinkles:- starts forming in early thirties and becomes evident in early forties.
For fine localized wrinkles , fillers can be used.

For generalized ones:- facial resurfacing with Chemical Peel, Dermabrasion or Laser is best suitable option. For generalized wrinkles with sagging face :- face lift with or without resurfacing.

Double Lip / Thick Lip surgery :-   Double Lip is a variant of occult ( Hidden ) Cleft lip, occurs due to mal -alignment of muscles acting on lip. Correction needs both removal of excess fibro-fatty-vascular tissue as well as restoration of normal lip muscle orientation and frenum lengthening.

         Thick lip usually lower lip, often , is due to small receded chin  which alters axis of pull of muscles and leads to evesrion of mucosa,  so correction involves both Lip reduction, Chin Enlargement or Augmentation and correction of  orientation of  muscles.

                                        This is important in plastic & Cosmetic Surgery to understand the reason behind any problem, Simplest  treatment may not be the optimal treatment always.
Double chin:- accumulation of fat in subcutaneous plane. Liposuction wit or without platysmaplasty is treatment.
Sagging neck:- due to accumulation of fat as well as laxity of neck muscle platysma.

Chin augmentation (genioplasty):- to augment small chin can be done with implant either medpore or silicone or with advancement of chin bone.

Profiloplasty:- side view of face is called profile view. It shows projections and depressions of Face. nose and chin affect the profile view most.  Augmentation or reduction may be needed to provide a beautiful appearance.
