In Men, it hides visible and useful length of penis and
so affects personal life, in females , it gives genitalia

In early , phase removal of fat through liposuction can
restore youthful appearance but when associated with
excess loose skin , mons lift is done to restore the same.
Combined procedures :- liposuction,

combination to get best result in more severe obese cases
with loose skin as it both tightens abdominal wall as well
as lifts sagging vagina .

Labiaplasty
:- an aesthetically pleasing labia
minora is usually hidden inside labia majora and visible
only when outer lips are pulled apart. With increasing age
and increasing peripheral androgen activity in females ,
labia minora enlarges and stats protruding beyond the
labia majora.
Enlarged labia minora , not only gives genitalia an
unsightly look but also interferes with maintenance of
hygiene and difficulty in wearing well fitting cloths.

Clitoris unhooding /
Hoodectomy :-
clitoris is a structure
homologous to penis in males. In males, glans is usually
uncovered or the prepuce can be retracted back to expose
it. In some cases, circumcision is needed to uncover it.
On contrary, in females, glans of clitoris is usually
covered with prepuce.
In unhooding procedure, overhanging skin is removed with
either a circumcision type incison or with an incision at
base of clitoris to expose the glans clitoris.
It drastically improves sexual sensation and pleasure of
love making. Clitoris unhooding or clitoris hoodectomy
increases frequency of orgasm in females.
Clitoromegaly :-
means enlarged clitoris, occurs due
to increased male sex hormone in female body, either due
to some congenital deformity or with increasing age.

Hymenoplasty or Hymen Repair
or Revirgination Surgery
:-
vagina is formed in embryonic
life by two different sources, mullarian duct and
sinovaginal bulb, hymen is the junction point of these two
structures. Hymen is a membrane which is partly absorbed in
center to form a passage for exit of menstrual blood.
Hymen is considered a sign of virginity in females, can be lost
because of several reasons including intercourse. With
Hymenoplasty or Hymen Repair or Re-virgination procedure, Hymen
can be restored again to give a virgin look along with
other changes, whenever needed.
Two types of
surgery available -
Hymenorrhaphy :-
for
short term result , done 7-10 days before marriage ,
this surgery is based on suture approximation of hymenal
tags which cuts through with force of penetration.
During examination hymen repair is identifiable.
for natural looking hymen
- Hymen repair or reconstruction should be done at
least six weeks before marriage, or as early as
possible. after proper healing hymen looks , feels and
behaves like natural hymen. Its a common misconception
that hymen never heals so you are bound to rely upon
sutures , its not true, it heals well with proper
surgery and if given proper time and care. Surgical time
( 30-40 minutes ), recovery time (few hours- may be
little pain or discomfort ) and rest (5-6 hours) ,
all are same , only you need to get it done at least 6
weeks before marriage and dont part your vaginal lips
for at least three weeks.

Pathologies of vulva :-
Various skin conditions (like lymphangioma, other tumors,
cancers and post infective pathologies) may need excision
and reconstruction of vulva (female external genitalia).

Lymphangioma ( a pathological condition manifesting
as multiple small vesicles (swellings) discharging white
fluid- Its a Developmental deformity and not any
infection. Patient can lead normal married life after

Neo-vaginoplasty
(or Vagina Reconstruction):-
Creation of a new vagina. Done in cases with
congenital ( by birth) absence of vagina or vaginal atresia
or Aplasia.
Some cases are having vaginal canal but that is very
small or hypoplastic (underdeveloped or very narrow ,
unable to allow intercourse). There are
syndromes ( constellation of symptoms involving different
organ systems), which affect uro –genital and other
systems of body , absent vagina or vaginal atresia may be the only or the one
of features in such patients.
Mayer-Rokitansky-Kuster-Hauser
syndrome is most common
Syndrome with vaginal atresia . Two Types are there, In,
Type
I Mayer-Rokitansky-Kuster-Hauser
syndrome
(MRKH Syndrome) is characterized by an isolated
absence of the proximal two thirds of the vagina,
whereas Type II
Type I Mayer-Rokitansky-Kuster-Hauser
syndrome
(MRKH Syndrome) is marked by other malformations; these
include vertebral, cardiac, urologic (upper tract), and
otologic (Ear) anomalies.
So these patients should be investigated for other
congenital malformations and should be dealt
accordingly, if any. Proper history and some
radiological investigations are all that is required.
Absent uterus cant be created , but vagina reconstruction can be
done
so that patient can have a successful married life.
While doing vagina reconstruction or Vaginoplasty, characteristics of a normal vagina
is kept in mind and one or a combination of various
options of vagina reconstruction or vaginoplasty are used to create it.
Normal features of vagina:-
A tunnel which should accommodates usual size penis (
about 6 inches)
New Vagina
Should be warm and moist
New Vagina should be
Stretchable
New Vagina
Should secrete fluid upon stimulation which acts as
lubricant.
New vagina
Should be sensate (having both protective and erotic
sensation)
Options available for Neo-Vaginoplasty:-
Gradual Dilatation :- takes long time and little
cumbersome to patient but gives natural vagina without any
scar in any other area of body.
Split thickness skin graft:- split thickness skin
graft is harvested from thigh or buttock and a tube is
formed with it for vagina reconstruction or Vaginoplasty (McIndoe
Vaginoplasty). Needs dilatation for around six months
thereafter.
Full thickness skin graft:- taken from groin crease
for vagina reconstruction or Vaginoplasty. chances of re-contracture are less.
Local skin flaps:- from area adjacent to external
genitalia for vagina reconstruction or vaginoplasty.
Mucosal Grafts to create new
vagina - one of the
techniques which gives most natural appearance as well
as functional competence.

Labia minora flap:- labia minora is used to create
the Vaginal canal ( vagina reconstruction or Vaginoplasty), gives much more satisfactory result
but with little
alteration of external genitalia. Needs dilatation for a
few months. We don't advocate any distortion of external
genital appearance.
Local flap preserving all details of external
genitalia:- this technique of vagina
reconstruction or vaginoplasty uses local tissue from
external genitalia , but preserves all details of vagina
or external
genitalia and provides a vagina which is near natural and
no added scar anywhere on body.

Sex Change or Gender
Reassignment Surgery or Sex Reassignment Surgery (SRS):-
Male to
Female Sex Change Surgery :- various surgeries are required
including vagina & Vulva reconstruction, Breast
reconstruction, Changes in facial & other body features to
give feminine appearance as per requirement and desire of
patient.
Female to Male Sex Change Surgery :-
surgery includes de-feminizing surgery ( removal of
breast, removal of uterus & internal female genital and
gonads, obliteration of vagina & finally reconstruction of
penis.
Penis (Phallus
) Reconstruction ( Phalloplasty) :- needed in cases
of traumatic loss of penis, rare congenital absence of
penis (Aphalia) , in cases of very small penis (
micro-penis or micro phallia), after surgical removal of
penis and in cases of sex change or gender or sex
reassignment surgery. we have various techniques for it
including hormonal stimulation of clitoris followed by
volume & legth augmentation as in cases of sex change or
gender reassignment surgery ( SRS) , or complete
reconstruction of penis using various flaps. |
